Thank you for letting us know that you need to cancel a lesson.  Please complete the form below.  We will do our best to accommodate your requested date for a make-up lesson.  Someone will contact you to reschedule the lesson.   Please DO NOT make plans until an OFH representative has communicated with you to confirm the date and time of your make-up lesson.  All make-up lessons must be completed in your current four week session.
