SESSION REQUEST FORM
OFH Sessions are sold as a four-week package. The session includes four weekly small group one-hour lessons with no more than three hitters per class. You will keep the same time slot for the entire session. All lessons, including make up classes must be completed the four week session. Cost for a session is $250.
Please complete this form to request a time slot for a 4-week session. We will do our best to accommodate your first choice. Sessions fill up quickly. Please give us an alternative time slot(s) in addition to your first choice.
